Solemn declaration concerning a citizenship certificate that was lost, stolen, destroyed or never received

This form is for Canadian citizens who:

  • applied for a citizenship certificate and six weeks have passed since the certificate was mailed to them. To know when your certificate was mailed, you can access your file online, or contact the CIC Call Centre.

or

  • possessed a Canadian citizenship certificate that was lost, stolen or destroyed but who do not wish to reapply for a new certificate immediately.

You may also choose to apply for a new citizenship certificate.
